Best Mattress for Back Sleepers (2026): Medium-Firm Beds That Hold Your Lumbar Curve

Back sleeping is the position where a mattress has the most direct effect on spinal shape. We ranked every back-friendly bed in our database by RTINGS’ back-sleeper scores, measured firmness, spinal-support data and lumbar zoning.

By Kevin, writer & researcher17 min read🔬 Research-based, not sponsored

The short verdict: the best mattress for back sleepers

The best mattress for back sleepers is the Saatva Latex Hybrid. It combines a zoned coil unit with organic latex, RTINGS scored it 7.6 for back sleepers and measured spinal support of 7.7, and Sleep Foundation names it best for back sleepers under 230 lb. It also has the best longevity score in our database (8.3), which matters more for back sleepers than anyone else: a mattress that sags in the middle bends a supine spine every single night.

If $2,346 is too much, the DreamCloud Classic Hybrid at about $699 is the best value: Sleepopolis and Mattress Clarity both call it a back-sleeper bed, and RTINGS scored it 7.1 for back sleepers. If you split the night between your back and your side, choose the Bear Elite Hybrid in Medium, RTINGS’ best medium-firm mattress of 2026. And if you weigh over 230 lb, go straight to the Saatva HD, whose spinal-support score of 8.3 is the highest we track.

This page is about sleep position. If you are shopping because your back already hurts, read our best mattress for back pain guide first — it walks through the Kovacs trial and the systematic reviews in detail. Here we focus on what a healthy back sleeper needs to keep it that way.

What back sleepers need from a mattress

Lie on your back on the floor and slide a hand under your waist. That gap is your lumbar lordosis, the natural inward curve of the lower spine. A good mattress for back sleeping does two things at once: it lets the two heaviest points — the shoulder blades and the buttocks — sink in just a little, and it rises to fill that gap under the lower back so the curve rests on something rather than hanging in the air. Too firm, and the bed pushes the hips up and flattens the curve. Too soft, and the pelvis sinks lower than the chest, so the spine sags into a shallow hammock.

That is not just theory. In 2022, Hong and colleagues put 16 adults on three mattresses of different stiffness and measured their spines while they lay on their backs. On the hard mattress, lumbar lordosis distance dropped to 11.6 mm, against 22.2 mm on the medium bed — the hard surface flattened the lower back, and peak contact pressures at the shoulder blades and buttocks were typically three to four times higher. The soft mattress kept the lumbar curve (22.9 mm), but the torso tilted further (6.86° against 3.64° on medium) and the head position rose by 30.5 mm, which raised the modelled load on the neck. Medium was the only surface that did well on both counts.

This lines up with the wider literature. The 2021 Caggiari review of 39 studies concluded that medium-firm mattresses promote comfort, sleep quality and spinal alignment, and the Kovacs trial in The Lancet found medium-firm beds beat firm ones for people with chronic low-back pain. Neither was limited to back sleepers, but the mechanism is easiest to see in this position, because your spine lies directly along the mattress surface rather than across it.

So the target is clear: a medium-firm bed with a properly supportive core. Two numbers in our data capture that best. The first is the spinal-support sub-score, which reflects how well a bed keeps the spine level under load. The second is RTINGS’ back-sleeper score, which combines alignment and pressure for a person lying supine. We also weighed durability more heavily than on our side-sleeper page, because a back sleeper’s hips sit in the same spot every night and are the first thing to find a dip.

Back sleeper mattress firmness: what level is right for your weight?

Firmness is relative to the body on top of it. A 120 lb person barely compresses a mattress’s comfort layers, so a bed labelled medium feels supportive and fairly firm. A 260 lb person sinks through those layers and onto the support core, so the same bed can feel medium-soft. That is why Sleep Foundation’s guidance for back sleepers shifts by roughly two points across weight groups: soft (3) to medium (5) under 130 lb, medium (5) to firm (7) between 130 and 230 lb, and firm (7–8) to extra firm (9) over 230 lb. Its firmness guide sums up the target in one line: back sleepers “generally require a slightly firmer mattress that resists sagging in the midsection.”

Back sleeper mattress firmness by body weight

Bands: Sleep Foundation firmness guidance for back sleepers. Dots: our picks’ consensus firmness from lib data (1 = softest, 10 = firmest).

Under 130 lb

Soft 3 – Medium 5 — You barely compress the comfort layers; medium already feels supportive.

130–230 lb

Medium 5 – Firm 7 — Most back sleepers. Medium-firm (6–7) sits in the middle of this band.

Over 230 lb

Firm 7 – Extra firm 9 — You sink further, so a firmer, coil-based core keeps hips level.

Our picks

1 soft5 medium10 extra firm

Three practical rules follow. First, most back sleepers should aim for 6 to 7 on the 1–10 scale — what retailers call medium-firm. Every one of our top four picks sits there. Second, don’t trust the label on the box. DreamCloud now calls the Classic Hybrid firm, yet RTINGS measured it at 39 Pa/mm, on the plush side of medium; Saatva sells the Latex Hybrid as medium-firm, and RTINGS measured it firm at 60 Pa/mm. We lean on measured firmness and tester ranges for that reason. Third, if you are under 130 lb, a bed rated 6.5 by average-weight testers can feel like a board. Mattress Clarity noted the Saatva Latex Hybrid is firm for lighter people, and Saatva Memory Foam Hybrid testers under 135 lb felt it around 8. Lighter back sleepers should lean towards the Saatva Rx or the Bear Elite Hybrid in Soft.

How do you tell if you got it wrong? On a bed that is too firm, back sleepers usually notice it at the tailbone and shoulder blades first, and feel a gap under the small of the back when they lie still. On a bed that is too soft, the sign is a dull ache across the lower back by morning, and a sense of rolling “uphill” to get out of bed. A partner can check alignment: lying on your back, your ear, shoulder, hip and ankle should line up roughly horizontally, with the hips not visibly lower than the shoulders.

Best firm mattress for back sleepers

If you know you prefer a firm surface, or you are over 230 lb, the Saatva Latex Hybrid is the firm mattress for back sleepers we would buy, because it gets its firmness from zoned latex and pocket coils rather than dense foam, so it stays buoyant and cool (RTINGS cooling 8.6). The budget alternative is the Bear Original: testers measured it from 6.5 to 8, Sleep Foundation calls it firm (7), and its support sub-score of 7.2 is higher than several beds that cost twice as much. Above 300 lb, the Saatva HD is the only firm bed in our database designed for the load.

Zoned lumbar support: does it help back sleepers?

A zoned mattress varies its support along its length. In the common three-zone layout, the area under the head and shoulders is a little softer, the area under the lower back and hips is firmer, and the leg section sits in between. For a back sleeper, the point is to stop the heaviest part of the body — the pelvis — from sinking further than the chest, while still letting the shoulder blades settle. That keeps the lumbar curve supported without making the whole bed hard.

How a zoned mattress supports a back sleeper

Typical three-zone layout, side view. The firmer middle zone holds up the heaviest part of your body so the lower back doesn’t hammock.

  • Saatva HD
Five-zone natural latex over heavy-gauge coils

The evidence is encouraging but thin. The Caggiari review reported that beds with customised zonal elasticity improved spinal alignment in the studies it included, but most of those studies were small, used healthy volunteers and tested research beds rather than retail mattresses. So treat zoning as a tie-breaker, not a reason to pay double. In our data, the beds with real zoning are also the ones with the highest back-sleeper and support scores — the Saatva Rx (RTINGS back 7.8), the Saatva Latex Hybrid (7.6) and the Saatva HD (support 8.3) — but they also have other strengths, so we can’t credit the zoning alone.

A practical point: a zoned bed only works if your lumbar area lands on the lumbar zone. Very tall or very short sleepers, and people who sleep far down the bed, may get less benefit. Rotating a zoned mattress head-to-foot is fine because the layout is usually symmetrical, but check the manufacturer’s guidance; flipping a one-sided bed upside down is not.

Back sleeper mattress scores compared

RTINGS publishes a separate score for back sleepers on every mattress it tests. Here is every RTINGS back-sleeper score we hold for the beds in our database. Above 7.5 is strong; below 7 means a meaningful share of back sleepers will notice either pressure or sag.

The pattern is the reverse of our side-sleeper chart. Beds that win for side sleepers — the plush DreamCloud Premier Hybrid and Puffy Lux — are absent here because RTINGS rates them too soft for most back sleepers. Medium-firm foam beds such as the Nectar Classic, which side sleepers should skip, sit at a respectable 7.0. And the Saatva Classic in Luxury Firm, despite its awards, scores only 6.7 for back sleepers at RTINGS; its appeal is feel, service and edges more than measured alignment.

6.5/10 · Medium-firm — Brand says Medium Firm (6), ‘best suited for back and stomach sleepers’; RTINGS measured Firm (60 Pa/mm).

The Saatva Latex Hybrid is the bed whose data lines up most neatly with what a back sleeper needs. RTINGS scored it 7.6 for back sleepers, measured spinal support of 7.7, and named it its best latex, best organic and best mattress for seniors in 2026. Sleep Foundation scored it 9.6 and says it is best for back sleepers under 230 lb; Tom’s Guide gave it 4.75/5.

Underneath its organic latex comfort layer is a zoned coil unit, which is what keeps the hips level while the latex supports the lumbar curve. It is also the bed least likely to develop the midsection dip that ruins back sleeping: longevity of 8.3 is the best in our database, against 5.1–5.7 for the Saatva Classic. Add cooling of 8.6 and edges of 9.1 and you have the most complete bed here. The catches are price (about $2,346 plus a $99 return fee), one firmness only, and a firm feel that lighter people and side sleepers will not love — RTINGS scored lightweight side sleepers just 6.1. More in our Saatva review.

6.5/10 · Medium-firm — Testers 6–7.5; brand now labels it Firm; RTINGS measured Medium-Plush (39 Pa/mm).

For about $699 a queen, the DreamCloud Classic Hybrid is the cheapest bed in our database that experts consistently recommend for back sleepers. Sleepopolis scored it 4.9/5 and calls it best for back, hot and combination sleepers; Mattress Clarity found it firmer than advertised and great for back sleepers and couples; Sleep Foundation says it suits back sleepers under 230 lb. RTINGS scored it 7.1 for back sleepers and measured edges of 9.0 and responsiveness of 9.0, so it is easy to move on and sit on.

Two cautions. Firmness reports range from 6 to 7.5 and RTINGS measured it on the plush side of medium, so it is near the soft limit for a back sleeper; if you are over 230 lb, consider the Bear Elite instead. And longevity is 5.5, so register the Forever warranty and rotate the bed. Our DreamCloud Classic Hybrid review and Nectar vs DreamCloud comparison go deeper.

6/10 · Medium-firm — Medium tested (RTINGS Medium-Firm, 51 Pa/mm). Soft ≈5, Firm ≈7–8.

Most back sleepers spend part of the night on their side, and the Bear Elite Hybrid is the bed that handles both best. RTINGS measured the Medium version at 51 Pa/mm, squarely medium-firm, and named it the best medium-firm mattress for most people in 2026. Its spinal-support score of 7.5 is high for a boxed hybrid, Forbes Vetted rated its pressure relief 9/10, and responsiveness of 9.4 means rolling from your back onto your side doesn’t feel like climbing out of a hole.

The three firmnesses let you tune it: Sleep Foundation rated them Soft 5, Medium 6 and Firm 8. Back and side combo sleepers under 230 lb should pick Medium; lighter combo sleepers Soft; back and stomach sleepers Firm. Edge support of 9.7 is the best in our database, and Tom’s Guide gave it its 2025 Best Cooling award. The weak spot is longevity at 4.3 — the lowest of our picks — so this is the bed to choose for comfort and versatility, not for decades of use. See our Bear review.

6.5/10 · Medium-firm — Luxury Firm (most popular). Also Plush Soft (Saatva’s label ~3; RTINGS measured 39 Pa/mm, medium-plush) and Firm (~8). RTINGS’ 43 Pa/mm Luxury Firm reading is from a double-pocket-coil unit; the official continuous-coil build tested medium-firm.

If you want the lifted, springy feel of a good hotel bed, the Saatva Classic in Luxury Firm is the one to buy. Sleepopolis measured Luxury Firm at 7.5/10, Tom’s Guide ranks it #1 in its overall guide, and Forbes Vetted names it its top mattress for back pain. Its coil-on-coil build is bouncy and cool (Sleep Foundation gave temperature control 10/10), and edges of 8.2 make getting up easy.

The honest caveat is that the lab data is weaker than the awards. RTINGS scored Luxury Firm 6.7 for back sleepers, our support sub-score is 6.5, and RTINGS found two coil designs sold under the same name. You are paying for feel, choice (three firmnesses, two heights) and white-glove delivery with old-bed removal. If measured support is your priority, the Latex Hybrid is the better Saatva. Compare them in our Saatva vs DreamCloud breakdown.

5/10 · Medium — Brand ‘Supportive Plush’ (3–4), ideal for side, back and combination sleepers (stomach sleepers only under 130 lb); testers range 4–6.5; RTINGS measured Medium and classifies it as an innerspring (two coil layers).

The Saatva Rx has the highest RTINGS back-sleeper score in our data (7.8) and the most targeted lumbar zoning of any bed here: a band of gel memory foam over micro-coils right where the lower back lands. RTINGS measured pressure relief of 8.0 and support of 7.7, and its side (7.9) and stomach (7.7) scores are almost as high, so it suits restless sleepers too. Tom’s Guide recommends it over the Saatva Classic for chronic back pain.

We rank it fifth for back sleepers in general because it is a specialist bed with a specialist price — about $3,094 for a queen — and edges of 5.3 and longevity of 5.0. If you are a back sleeper with lower back pain, it moves to the top of your list; our back pain guide covers it alongside the clinical evidence. If you are a back sleeper without pain, the Latex Hybrid gives you similar support for less.

7/10 · Firm — Saatva rates it Medium Firm (5–7) for sleepers over 300 lb and says those under 300 lb ‘might find it firmer’; average-weight testers measured 7–8.

Heavier back sleepers have the hardest time keeping their hips level, because the pelvis is where extra weight sits and where cheaper foams wear out first. The Saatva HD is built for 300 to 500 lb, with heavy-gauge coils under a five-zone natural latex layer, and its spinal-support sub-score of 8.3 is the highest in our database. Sleep Foundation scored it 9.2 with durability of 8.5, Sleepopolis measured it 7/10 firm, and TechRadar names it its best premium mattress for heavy people.

It is firm by design: Mattress Clarity’s average-weight testers felt it at 8/10, so anyone under 230 lb should look elsewhere. Between 230 and 300 lb, the Saatva Latex Hybrid or the Bear Elite Hybrid in Firm are reasonable middle grounds. Saatva recommends its HD foundation, which adds cost. For more options, see best mattress for heavy people.

7/10 · Firm — Testers 6.5–8; most say firm.

The Bear Original is a firm gel memory foam bed that costs about $699 on sale. Its support sub-score of 7.2 beats several hybrids at twice the price, Sleep Foundation ranks it its #2 cheap mattress, and testers recommend it for back and stomach sleepers. It is the bed for a back sleeper who likes a flat, firm surface and wants to spend as little as possible.

At 6.5 to 8 it is firmer than the medium-firm range the research favours, pressure relief is only 6.0, and it sleeps warm (cooling 5.5) with weak edges. If you sometimes turn onto your side, the DreamCloud Classic Hybrid is the better budget bed. More cheap options are in our best affordable mattress guide.

What expert reviewers concluded

Best mattress for back and side sleepers, and back and stomach sleepers

Few people stay on their back all night — Skarpsno’s participants changed position about 1.6 times an hour on average. So the useful question is not just “am I a back sleeper?” but “what is my second position?” That changes the right firmness by a point or so either way.

Back and side sleepers need a little more give at the shoulder than a pure back sleeper, without letting the waist sag when they roll back. That means the softer end of medium-firm (around 6) and a responsive hybrid rather than slow memory foam. The Bear Elite Hybrid in Medium is our pick; the Saatva Rx is the premium alternative with the best side score here (7.9). If you spend more than half the night on your side, our best mattress for side sleepers guide is the better starting point.

Back and stomach sleepers should go the other way. Face-down, the hips are the lowest point, and any sink arches the lower back, so stomach sleeping punishes a soft bed more than any other position. Aim for 7 or firmer. The Saatva Latex Hybrid (RTINGS stomach 7.7) is the pick, with the Bear Original or the Bear Elite in Firm as cheaper routes. Our best mattress for stomach sleepers guide has the full list.

Couples with one back sleeper should also think about motion. The Saatva Classic and Saatva Latex Hybrid are bouncy coil beds, and Mattress Clarity scored the Classic’s motion isolation just 2.6/5. If your partner is a light sleeper, the Nectar Classic Hybrid (motion isolation 8.6, RTINGS back 6.9, about $849) is a medium-firm compromise both of you can share. See best mattress for couples.

Hot back sleepers are well served by this list, because medium-firm coil beds sleep cooler than plush foam. The Saatva Latex Hybrid (8.6), DreamCloud Classic Hybrid (7.9) and Bear Elite Hybrid (7.9) all measured well for cooling. Avoid thick all-foam beds if heat is a complaint; the Bear Original’s cooling of 5.5 is the weak point of an otherwise good budget bed. Our best cooling mattress guide goes further.

Back sleeping, snoring and sleep apnea

There is one health reason not to sleep on your back, and it has nothing to do with the mattress. For many people, snoring and breathing pauses get worse when they lie on their back. A systematic review by Menon and Kumar (2013) found that supine sleep posture was consistently associated with more severe obstructive sleep apnea indices in adults. In a study of 120 patients with sleep apnea, Richard and colleagues (2006) found that 55.8% had position-dependent apnea, meaning their breathing events were at least twice as frequent on their back as in other positions.

That matters for mattress shopping in two ways. First, if your partner says you snore loudly or stop breathing at night, or you wake unrefreshed despite a full night in bed, speak to a doctor before you spend money on a new bed — sleep apnea is a medical condition, and no mattress fixes it. Second, if you have been diagnosed with position-dependent apnea, a review by Ravesloot and colleagues (2013) describes positional therapy — devices that discourage back sleeping — as a useful option for many patients. In that case you may be training yourself into a side sleeper, and a bed that suits both positions (the Bear Elite Hybrid or Saatva Rx) is a safer buy than a firm back-only bed.

Back sleepers who simply snore lightly sometimes find a slightly higher pillow helps, but don’t overdo it: Hong’s data showed a soft mattress raising the head about 30 mm relative to medium, and extra neck flexion brings its own problems. A pillow for a back sleeper should support the neck’s curve while keeping the head roughly level with the spine.

Mattresses back sleepers should skip

These beds are good at other jobs, but the data says they are the wrong choice if you mainly sleep on your back:

  • DreamCloud Premier Hybrid — our best side-sleeper bed, but RTINGS measured it medium-plush (36 Pa/mm) and says it is too soft for most back and stomach sleepers.
  • Puffy Lux Hybrid — testers measured it at 5–5.5, softer than its label, and Tom’s Guide had doubts about its support. Great for light side sleepers; wrong for back sleepers.
  • DreamCloud Luxe Hybrid — a 14-inch pillow-top that RTINGS rates weak for back and stomach sleeping.
  • Nectar Luxe and Nectar Premier — deep foam built for side sleepers; Mattress Clarity says the Premier is weak for stomach sleepers, and both are too soft for heavier back sleepers.
  • Saatva HD — unless you weigh over 230 lb. Average-weight testers found it around 8/10.
  • Any mattress with a visible dip in the middle. Sleep Foundation suggests most mattresses last about 6 to 8 years. A sag under the hips undoes everything on this page.

Not on the skip list but worth a note: the Saatva Memory Foam Hybrid (support 7.2, medium-firm 6.5) is a decent back-sleeper bed that Tom’s Guide recommends mainly for back sleepers, and the Nectar Classic (RTINGS back 7.0, about $699) is a fine cheap foam option if you don’t mind slow response and soft edges. Neither beats the picks above at its price. For all-round winners, see our best mattress roundup or take the 30-second mattress quiz.

Finally, use the trial properly. Every pick on this page has a 365-night trial. Give a new bed three to four weeks before judging it, note how your lower back feels on rising each morning, and if it is no better after a month, return it and move one firmness step — firmer if your hips feel low, softer if your tailbone and shoulder blades ache.

Back sleeper mattress FAQ

What is the best mattress for back sleepers?

The Saatva Latex Hybrid for most back sleepers. RTINGS scored it 7.6 for back sleepers and 7.7 for spinal support, it uses a zoned coil unit, and its longevity score of 8.3 is the best we track. On a budget, the DreamCloud Classic Hybrid at about $699.

What firmness is best for back sleepers?

Usually medium-firm, around 6 to 7 out of 10. Sleep Foundation’s guidance for back sleepers is soft (3) to medium (5) under 130 lb, medium (5) to firm (7) between 130 and 230 lb, and firm (7–8) to extra firm (9) over 230 lb.

Is a firm mattress good for back sleepers?

Firm suits heavier back sleepers and back-and-stomach sleepers. For average weights, too firm can flatten the lower back: in Hong 2022, lumbar lordosis distance was 11.6 mm on a hard mattress versus 22.2 mm on a medium one, with much higher contact pressure. Medium-firm is the safer default.

What is the best mattress for back and side sleepers?

The Bear Elite Hybrid in Medium. RTINGS measured it medium-firm (51 Pa/mm) and named it the best medium-firm mattress of 2026; responsiveness of 9.4 makes switching positions easy. The Saatva Rx is the premium alternative (RTINGS back 7.8, side 7.9).

What is the best mattress for heavy back sleepers?

The Saatva HD, rated to 500 lb per side. Its spinal-support score of 8.3 is the highest in our database, and Sleep Foundation scored its durability 8.5. Between 230 and 300 lb, the Saatva Latex Hybrid or Bear Elite Hybrid in Firm also work.

What is the best mattress for back sleepers with lower back pain?

A medium-firm bed with lumbar support. The Saatva Rx has a dedicated lumbar zone and RTINGS’ highest back-sleeper score in our data (7.8). Our best mattress for back pain guide covers the clinical evidence and more picks.

Is sleeping on your back bad for snoring?

It can be. A 2013 systematic review found supine sleep consistently linked to more severe obstructive sleep apnea, and in one study of 120 patients, 55.8% had position-dependent apnea. If you snore loudly or stop breathing at night, see a doctor — a mattress won’t fix it.

Do back sleepers need a zoned mattress?

It helps but isn’t essential. Zoned beds are firmer under the hips and softer under the shoulders, which keeps the lumbar curve supported. Research on zoning is small-scale, so get firmness right first and treat zoning as a tie-breaker.
