The short verdict: the best mattress for back pain
The best mattress for back pain is a medium-firm one, and the best medium-firm mattress we can find is the Bear Elite Hybrid in its Medium option. RTINGS measured it as medium-firm (51 Pa/mm) and named it the best medium-firm mattress for most people in 2026. Its spinal-support sub-score of 7.5 is among the highest of any boxed hybrid we track, and you can order it Soft or Firm if your weight or sleep position calls for it.
If you live with chronic back and hip pain and can afford it, the Saatva Rx is the specialist pick: lumbar zoning, pressure relief of 8.0, support of 7.7, and RTINGS positional scores of 7.9 for side sleepers and 7.8 for back sleepers. If you want a firm mattress for back pain that will last, the Saatva Latex Hybrid pairs support of 7.7 with the best longevity score we track (8.3). And for under $900, the Nectar Classic Hybrid is the best-value medium-firm bed.

What the science says about mattresses and back pain
The mattress industry has spent decades telling people with bad backs to buy the firmest bed they can find. The best evidence we have says that is wrong — or at least, not right for most people.
The landmark study is Kovacs and colleagues, published in The Lancet in 2003. Researchers randomly assigned 313 adults with chronic, non-specific low-back pain to sleep on either a firm mattress or a medium-firm one, without telling them which they had, and followed them for 90 days. The medium-firm group did better on all three main outcomes. Their odds of improvement were 2.36 times higher for pain while lying in bed, 1.93 times higher for pain on rising, and 2.10 times higher for disability. The pain-on-rising result narrowly missed statistical significance; the other two did not.
Kovacs 2003 (Lancet): odds of improvement on medium-firm vs firm
313 adults with chronic low-back pain, 90 days. Dot = odds ratio; line = 95% confidence interval. Right of the dashed line favours medium-firm.
One detail trips people up. Kovacs used the European H-scale, where lower numbers mean firmer — the opposite of the 1-to-10 scale mattress reviewers use. The firm mattresses were rated H 2.3 and the medium-firm ones H 5.6. In everyday terms, that is the difference between a genuinely hard bed and one with some give at the surface but solid support underneath. The two scales don’t convert exactly, so we use RTINGS’ objective firmness readings to identify retail beds that fall in the medium-firm range.
The reviews point the same way. A 2021 systematic review by Caggiari and colleagues in the Journal of Orthopaedics and Traumatology pooled 39 studies and concluded that “a medium-firm mattress promotes comfort, sleep quality and rachis alignment” — rachis meaning the spine. An earlier systematic review by Radwan and colleagues in Sleep Health (2015), which looked at 24 controlled trials published since 2000, reached a similar conclusion: a mattress that people rated as medium-firm and that could be custom-inflated, or self-adjusted, was optimal for comfort, sleep quality and spinal alignment.
And then there is the study you should take with a pinch of salt. Jacobson and colleagues (Journal of Chiropractic Medicine, 2009) had 59 adults sleep on their own beds for 28 days and then on new medium-firm bedding for another 28 days, and found significant improvements in sleep quality and back discomfort. That is often quoted as proof that a new mattress fixes back pain. But there was no control group, the old beds averaged about 9.5 years old, and the paper states that its funding came from the Better Sleep Council, the mattress industry’s consumer-education group. It tells you an old, worn mattress is probably worse than a new one. It does not tell you which new one to buy.

Medium-firm vs firm mattress for lower back pain
Why would a medium-firm mattress beat a firm one? The leading explanation is about how your spine sits. Your body is not flat: the shoulders and hips stick out, and the waist and lower back curve in. On a very firm surface, the heaviest points press hard into the bed while the lower back is left unsupported, so muscles stay tense to hold your position. On a medium-firm bed, the shoulders and hips sink in slightly, the mattress comes up to meet the curve of the lower back, and the spine can rest in a more neutral line.
Too soft is just as bad in the other direction. If the hips sink much further than the shoulders, the lower back bends into a hammock shape — which is why sagging old mattresses so often cause morning stiffness. That is where support comes in. In our data, spinal support is a separate sub-score from firmness: a bed can feel soft on top and still hold your hips level, or feel hard and still let the middle sag over time.
When is a firm mattress better for back pain? Three situations. If you sleep on your stomach, a firm bed stops your hips sinking and arching your lower back. If you weigh over 230 lb, a bed that measures medium-firm for an average person will feel medium under you, so going firmer gets you to the same place. And some people simply feel better on a firm bed — if that’s you after a fair trial, trust your back over a study average. For everyone else, start with medium-firm.
One more practical point: mattress labels are unreliable. The DreamCloud Classic Hybrid is now labeled firm by its brand, yet RTINGS measured it on the plush side of medium; the Saatva Latex Hybrid is sold as medium-firm but measured firm. That is why every pick on this page is based on measured firmness and independent tester ranges, not the number on the box.

Firm mattress for back pain: what the lab models show
The Kovacs trial tells you what happened to people’s pain. A smaller lab study helps explain why. In 2022, Hong and colleagues published a study in the journal Biology comparing soft, medium and hard foam mattresses. They measured the spinal curves of 16 volunteers lying on their backs, then built a computer model of a male body to estimate the pressure on the skin and the load on the discs between the vertebrae.
The hard mattress concentrated pressure. At the shoulder blades and buttocks — the points that stick out when you lie on your back — contact pressure on the hard mattress was roughly three to four times higher than on the medium one. It also flattened the natural inward curve of the lower back slightly compared with the medium bed. The soft mattress had the opposite problem: the body sank and the spine bent, and peak loading on a disc in the neck rose by 49% compared with the medium mattress. The authors concluded that the medium mattress was the most suitable of the three.

Keep this study in proportion. It used young volunteers, one body model, one pillow and one sleeping position, and it measured mechanics, not pain. But it lines up neatly with the clinical trials: the best firm mattress for back pain is usually one that is firm underneath and slightly forgiving on top, not a board. That is why our firm pick, the Saatva Latex Hybrid, pairs a measured firm feel with a five-zone latex comfort layer, and why we would rather see a medium-firm hybrid with high support scores than an extra-firm foam bed for most people. If you sleep on your stomach, see our best mattress for stomach sleepers guide, where firm genuinely is the right call.
Do zoned, adjustable or air mattresses help back pain?
Both systematic reviews found hints that adjustability matters as much as firmness. Radwan’s review concluded that the best results came from a mattress people rated medium-firm and could custom-inflate, meaning sleepers could tune it themselves. Caggiari’s review also reported that beds with active control or customised zonal elasticity improved spinal alignment and sleep quality in the studies it included.
In retail terms, there are three ways to get some of that benefit. Zoned support puts softer material under the shoulders and firmer material under the hips and lower back; the Saatva Rx (lumbar zone), Saatva Latex Hybrid (five-zone latex over individually wrapped coils) and Saatva HD (five-zone latex) all use it. Firmness choice lets you match the bed to your weight and position; the Bear Elite Hybrid comes in three firmnesses and the Saatva Classic in three firmnesses and two heights. Adjustable air beds come closest to the custom-inflated designs in the studies, but none are in our database yet, so we can’t rank them against the lab data used here.
A note of caution: the studies behind these findings were small, often used healthy volunteers, and tested research prototypes rather than mattresses you can buy. Zoning is a sensible feature to prefer when two beds are otherwise equal, not a reason to pay double. Getting firmness right and replacing a sagging bed will do far more for most backs than any marketing claim about zones.
Best mattresses for back and hip pain, compared
We ranked every bed in our database on the two sub-scores that matter most for back pain — spinal support and pressure relief — then checked RTINGS’ back-sleeper scores and expert back-pain awards. Durability matters too: a bed that sags in three years will give you back pain whatever it measured when new.

6/10 · Medium-firm — Medium tested (RTINGS Medium-Firm, 51 Pa/mm). Soft ≈5, Firm ≈7–8.
The Bear Elite Hybrid is our pick because it hits the firmness the evidence favours and backs it up with measured support. RTINGS measured the Medium version at 51 Pa/mm — squarely medium-firm — and named it its best medium-firm mattress for most people in 2026. Our spinal-support sub-score is 7.5, Forbes Vetted rated its pressure relief 9/10, and Tom’s Guide gave it its Best Cooling award in 2025.
The three firmness options are a real advantage for back pain. Sleep Foundation rated them Soft 5, Medium 6 and Firm 8: most back sleepers should choose Medium, heavier and stomach sleepers Firm, and side sleepers with back pain Soft. Edge support measured 9.7, the best in our database, which helps if pain makes getting out of bed difficult. The weakness is durability: RTINGS rated longevity only 4.3. Register the warranty, rotate the bed, and if it starts to dip, claim early. More in our Bear review.

5/10 · Medium — Brand ‘Supportive Plush’ (3–4), ideal for side, back and combination sleepers (stomach sleepers only under 130 lb); testers range 4–6.5; RTINGS measured Medium and classifies it as an innerspring (two coil layers).
The Saatva Rx is a therapeutic mattress built around people who already hurt. Micro-coils sit under a lumbar zone of gel memory foam, so the bed contours at the shoulders and hips while supporting the lower back. RTINGS measured pressure relief of 8.0 and support of 7.7, with even positional scores — 7.9 side, 7.8 back, 7.7 stomach — that make it the best choice for back and hip pain together. Tom’s Guide recommends it over the Saatva Classic for chronic back pain, and Sleepopolis gave motion isolation 5/5.
It is expensive at about $3,094 for a queen. RTINGS also measured edges of 5.3 and longevity of 5.0, and its firmness is debated: Saatva calls it supportive plush, testers measured anything from 4 to 6.5, and RTINGS rated it medium. Buy it if pain is the problem you are trying to solve and you’ll use the 365-night trial. If you just want a good bed for a mildly stiff back, the Bear Elite does most of the job for half the price.

6.5/10 · Medium-firm — Brand says Medium Firm (6), ‘best suited for back and stomach sleepers’; RTINGS measured Firm (60 Pa/mm).
If you know you prefer a firmer bed, or you sleep on your stomach, the Saatva Latex Hybrid is the firm mattress for back pain we would buy. RTINGS measured it as firm (60 Pa/mm), with support of 7.7 and back and stomach scores of 7.6 and 7.7. Sleep Foundation scored it 9.6 overall and names it best for back sleepers under 230 lb.
Its biggest advantage for back pain is durability. Sagging is one of the most common causes of mattress-related back pain, and this bed has the best longevity score in our database (8.3) thanks to its organic latex. It also sleeps cool (8.6) and has edges of 9.1. Lightweight side sleepers should avoid it; RTINGS scored them only 6.1. It costs about $2,346 with a $99 return fee.

Saatva Classic

6.5/10 · Medium-firm — Luxury Firm (most popular). Also Plush Soft (Saatva’s label ~3; RTINGS measured 39 Pa/mm, medium-plush) and Firm (~8). RTINGS’ 43 Pa/mm Luxury Firm reading is from a double-pocket-coil unit; the official continuous-coil build tested medium-firm.
The Saatva Classic has the most back-pain awards of any bed here: Forbes Vetted names it its top pick for back pain, and Tom’s Guide puts it #1 in its overall guide. In Luxury Firm it sits near medium-firm, and its coil-on-coil build gives a lifted, traditional feel that many people with stiff backs find easy to move on. RTINGS scored it 6.7 for back sleepers and 7.3 for side sleepers.
We rank it fourth because the lab data is less convincing than the awards. Our support sub-score is 6.5, below the beds above, and RTINGS gave longevity 5.1–5.7. Still, if you want an innerspring with white-glove setup, three firmness levels and two heights, it is a solid choice. Read the full Saatva review or Saatva vs DreamCloud.

6.5/10 · Medium-firm — Testers 5.5–7; RTINGS Medium-Firm (56 Pa/mm).
For under $900, the Nectar Classic Hybrid is the medium-firm bed we recommend. RTINGS measured it at 56 Pa/mm — firmly in medium-firm territory — with support of 7.1 and back and stomach scores of 6.9 and 7.0. Sleepopolis recommends it for back sleepers and couples, Sleep Foundation rated it firm (7), and its motion isolation (8.6) is excellent.
RTINGS flagged lower-quality memory foam and longevity of 5.7, and light side sleepers will find it too firm. But at around $849 with a 365-night trial and Nectar’s Forever warranty, it is a low-risk way to test whether medium-firm helps your back. The DreamCloud Classic Hybrid (~$699) is a close alternative with better edges but softer measured feel and lower support (6.8). Compare them in Nectar vs DreamCloud.

7/10 · Firm — Saatva rates it Medium Firm (5–7) for sleepers over 300 lb and says those under 300 lb ‘might find it firmer’; average-weight testers measured 7–8.
Heavier bodies sink further into any mattress, so a bed that is medium-firm for an average person can feel soft and let the hips drop. The Saatva HD is built for 300 to 500 lb, with heavy-gauge coils and a five-zone natural latex layer, and its spinal-support sub-score of 8.3 is the highest in our database. Sleep Foundation scored it 9.2 with 8.5 for durability; TechRadar names it the best premium mattress for heavy people.

7/10 · Firm — Testers 6.5–8; most say firm.
The Bear Original is a firm gel memory foam bed with a support sub-score of 7.2 — higher than several mattresses that cost twice as much — for about $699 on sale. Sleep Foundation ranks it its #2 cheap mattress, and Sleepopolis scored it 4.5/5. It suits back and stomach sleepers who like a firm, flat feel.
Testers measured it from 6.5 to 8, so it is firmer than the medium-firm the trials favour. Pressure relief is only 6.0, so side sleepers and anyone with hip or shoulder pain should skip it, and it sleeps warm (cooling 5.5). For more under-$1,000 options, see our best affordable mattress guide.

Best mattress for back pain by sleep position
Back sleepers with lower back pain are the group the Kovacs trial maps onto most directly. Choose medium-firm: the Bear Elite Hybrid in Medium, or the Nectar Classic Hybrid on a budget. A small pillow under the knees can take strain off the lower back.
Side sleepers with back pain need softer shoulders without a sagging waist. The Saatva Rx (side 7.9) is the best measured option. The Bear Elite Hybrid in Soft is the cheaper route. A pillow between the knees keeps the pelvis level. Our best mattress for side sleepers guide covers this in more depth.
Stomach sleepers should go firm. Sleeping face-down lets the hips sink and arches the lower back, so firmness matters more for you than for anyone else. The Saatva Latex Hybrid (stomach 7.7) or the budget Bear Original are the picks.
Combination sleepers should choose a responsive hybrid so turning over doesn’t mean climbing out of a hollow. The Bear Elite Hybrid (responsiveness 9.4 at RTINGS) and DreamCloud Classic Hybrid (9.0) are the easiest beds in our database to move on.
Best mattress for back and hip pain
Back and hip pain often travel together, and they pull a mattress in different directions. The hip wants a soft surface to sink into; the lower back wants the waist held level. The clinical trials above studied low-back pain, not hips, so the picks here lean on pressure-relief and positional data rather than trial results.
Start by working out where the hip pain comes from. OrthoInfo, the patient site of the American Academy of Orthopaedic Surgeons, notes that with hip bursitis “the pain is worse at night, when lying on the affected hip.” If that sounds familiar, a firm bed is the worst choice for you, because it pushes straight back on the bony point of the hip. You need a comfort layer deep enough to spread that load — but on a supportive core, or the hip will sink and twist your lower back.
Our pick for back and hip pain is the Saatva Rx, because it is the only bed in our data that scores at least 7.7 for pressure relief, support, side sleeping and back sleeping at the same time. On a mid-range budget, choose the Bear Elite Hybrid in Soft if you are mostly a side sleeper and Medium if you mostly sleep on your back. If you are under 130 lb and the hip is the bigger problem, a plush hybrid such as the DreamCloud Premier Hybrid or the Puffy Lux will relieve the hip better, but RTINGS rates the Premier too soft for most back sleepers, so it is a hip-first choice, not a back-first one.
If your current mattress is under five years old, not sagging, and simply too firm at the hip, try a 2–3 inch topper before replacing it. Our best mattress topper guide covers which ones soften the hips without letting the lower back sag. Side sleepers with hip pain will find more picks in our side sleeper guide.
Best mattress for sciatica and scoliosis
Sciatica is pain that runs from the lower back down the leg, caused by irritation of the sciatic nerve. There are no mattress trials specific to sciatica that we could find, so the practical advice comes from spine specialists. Spine surgeon Peter Derman, writing for Spine-health, recommends a medium-firm mattress, citing the same research on low-back pain covered above, and suggests one with an extended money-back guarantee so you can test it. He also explains that a bed that is too firm stops the shoulders sinking, while one that is too soft lets the pelvis sag — either can throw the spine out of line.
Position matters as much as the mattress. Derman suggests that side sleepers put a pillow between the knees or thighs with the knees slightly bent, and that back sleepers slide a pillow under the knees until the lower back feels comfortable. For a mattress, that points to the same shortlist: the Bear Elite Hybrid in Medium, the Saatva Rx if the pain is chronic, or the Nectar Classic Hybrid on a budget. Combination sleepers should favour a responsive hybrid so that turning over doesn’t mean twisting out of a hollow.
Scoliosis has even less mattress research behind it. We found no trials of retail mattresses for people with scoliosis, and the spinal curves involved vary so much from person to person that a general firmness rule is shaky. What carries over from the evidence above is the direction of travel: avoid extremes, favour a medium to medium-firm bed with good support, and lean towards beds with firmness options or a long trial so you can adjust. If you are being treated for scoliosis, your specialist’s advice on positioning should come first.
Buying a mattress for a bad back: timing, size and trials
A few practical choices make a bigger difference than people expect. Size: if you share a bed and one of you has back pain, a bigger bed reduces how often you’re disturbed or cramped into an awkward position. Our best king size mattress guide covers the beds that hold up best in larger sizes. Position: if you are a dedicated back sleeper, our best mattress for back sleepers guide goes deeper on support for that position.
Timing: most of the beds on this page are discounted for much of the year, so there is rarely a reason to buy at list price. Our guide to the best time to buy a mattress explains when the deepest sales tend to land. Trial: treat the trial as part of the treatment. Track pain on rising each morning, as Kovacs did, and give it a month. If you are still unsure what to shortlist, our step-by-step how to choose a mattress guide walks through firmness, materials and budget. And if you want a cheaper medium-feel hybrid to try first, read our DreamCloud Classic Hybrid review — though note that it measured softer and less supportive than the Nectar Classic Hybrid.
Who should skip what
- Skip very plush beds if your lower back hurts — unless you are a lightweight side sleeper. The Puffy Lux and DreamCloud Premier Hybrid are excellent side-sleeper mattresses but RTINGS rates the Premier too soft for most back and stomach sleepers.
- Skip the Saatva HD and Bear Original if you are a light side sleeper. Both are firm, and pressure on the hip can turn into back pain by morning.
- Skip cheap all-foam beds if you are over 230 lb. They compress fastest under heavier bodies, which is exactly the sag that causes back pain.
- Skip any bed that is 8+ years old and dipping. Even the industry-funded Jacobson study makes this point: participants’ old beds averaged about 9.5 years. Sleep Foundation’s replacement guide suggests most mattresses last around 6 to 8 years.
Finally, use the trial properly. Give any new mattress at least three to four weeks before judging it — your back needs time to adjust. Keep a simple note each morning of pain on rising, the same measure Kovacs used. If it is no better after a month, return the bed and adjust firmness by a step. Back pain mattress FAQ
What is the best mattress for back pain?
A medium-firm mattress with strong support. Our pick is the Bear Elite Hybrid in Medium: RTINGS measured it medium-firm (51 Pa/mm) and named it the best medium-firm mattress for most people in 2026. For chronic back and hip pain, the Saatva Rx; for a firm, durable option, the Saatva Latex Hybrid.
Is a firm or medium-firm mattress better for lower back pain?
Medium-firm, according to the best evidence. In a randomised trial of 313 adults with chronic low-back pain (Kovacs, The Lancet, 2003), people on medium-firm mattresses had about twice the odds of improvement in pain in bed and disability compared with people on firm mattresses after 90 days.
What is the best mattress for back and hip pain?
The Saatva Rx. It combines lumbar zoning with pressure relief of 8.0 and support of 7.7, and RTINGS scored it 7.9 for side sleepers and 7.8 for back sleepers. On a lower budget, the Bear Elite Hybrid in Soft or Medium.
Is a firm mattress good for back pain?
For most people, a firm mattress is not better than medium-firm and may be worse. Firm makes sense if you sleep on your stomach, weigh over 230 lb, or know from experience that firm suits you. The Saatva Latex Hybrid is our pick for a firm mattress for back pain.
Can a mattress cause back pain?
An old or sagging mattress can contribute. In Jacobson’s 2009 study, people’s old beds averaged about 9.5 years and new medium-firm bedding improved back discomfort — though that study was industry-funded and had no control group. Persistent pain should be checked by a doctor.
How long does it take to adjust to a new mattress for back pain?
Usually two to four weeks. Most brands require you to keep a mattress at least 30 nights before returning it. Track pain on rising each morning; if there’s no improvement after a month, try a step firmer or softer.
Do chiropractors recommend firm mattresses?
Advice varies, but the research many clinicians cite — Kovacs 2003 and systematic reviews by Radwan (2015) and Caggiari (2021) — favours medium-firm over firm for comfort, sleep quality and spinal alignment.
